I personally, and/or our
faith based institution, acknowledge that the FCPC staff
and their attorneys will be sharing their attorneys’ legal
strategies and mental impressions with our clients, FCPC members and
interested parties. Florida
law as it relates to attorney-client and/or work product privilege
provides for confidentiality of all these matters by any party they are
shared with, as well as, excluding any individual or organizations from
attending or being party to this information.
I
personally, as well as our faith based institution agree to keep all
these legal strategies and mental impressions confidential unless
expressly we are granted permission in writing via certified mail
return receipt requested by the FCPC or their attorney, Frederick H.
Nelson, P.A., 234 North Westmonte Drive, Suite 3000, Altamonte Springs,
Florida 32714, (407) 925-4575. Furthermore, we will only share
information about the FCPC that is available to the public on the
FCPC’s web site, http://www.fcpcamerica.org/.
All other information and correspondence disseminated by the FCPC in
correspondence, electronically, or verbally is covered by this
Disclosure and Confidentiality agreement and I will not disclose it
without getting specific written permission from the FCPC. I will abide
by the terms of this FCPC Disclosure and Confidentiality agreement, and
that I understand that a violation of this FCPC Disclosure and
Confidentiality Agreement will subject me to penalties of perjury.
